This programme is restricted to those working in the geographies of the Devon, Somerset and Cornwall, Kent, Surrey and Sussex. What is Personalised Care and Social Prescribing?Personalised care represents a new relationship between people, professionals and the system. It happens when we make the most of the expertise, capacity and potential of people, families and communities. Personalised Care will benefit up to 2.5 million people by 2024, giving them the same choice and control over their mental and physical health that they have come to expect in every other aspect of their life. A one-size-fits-all health and care system simply cannot meet the increasing complexity of people’s needs and expectations. Personalised care is based on ‘what matters’ to people and their individual strengths and needs. See this video for more details. The NHS Long Term Plan says personalised care will become business as usual across the health and care system and Universal Personalised Care confirms how we will do it. Social prescribing is a key component of Universal Personalised Care. It is a way for local agencies to refer people to a link worker. Link workers give people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. They connect people to community groups and statutory services for practical and emotional support. Link workers also support existing community groups to be accessible and sustainable, and help people to start new groups, working collaboratively with all local partners. ALS are ideal for experienced leaders who need to address complex issues with a peer group who meets regularly and works collaboratively. We collaborate with a directory of expert ALS facilitators. We recommend groups of between six and eight leaders and where possible we will attempt to group according to your level of leadership experience. Each group will receive four facilitated sessions. Peer Journeys will offer groups of participants the opportunity to learn from each other while taking on real-world challenges and building leadership skills in the process. You will have the opportunity to take part in peer coaching, a practice which develops trust and strengthens the bonds between you and your peers. Substantial time will be spent in a peer group challenge, with deep dives into social prescribing practice. Groups and individuals will be expected to devise plans for how social prescribing can be improved in their own region and community. Coaching effectively guides you towards increasing your resilience, efficiency, and performance - getting you closer to achieving your personal and professional goals. We have a roster of expert coaches highly experienced in
social leadership, who can help you gain clarity, understand your vision and mission. We recommend four one-to-one sessions per leader, with an interval of four weeks between coaching sessions.
Learning days are online facilitated sessions which build confidence, collaboration, resilience and personal effectiveness. Learning days will cover a number of different leadership behaviours as well as give you a chance to interact with your fellow participants. ![]() What you'll learnThe programme learning will be guided by Clore Social Leadership's tried and trusted leadership development frameworks. Leadership is tough, so it is also important to build physical and emotional resilience, and look after their own wellbeing so they can respond effectively to leadership challenges. We live in a dynamic world that is transforming our workplaces, politics, ethics and communities. To meet these demands, social leaders need to understand the complexities of changing systems. They must also be aware of the challenges and opportunities that lie ahead to steer their teams and organisations, and make the most of what these changes offer. Social skills and empathy are required to inspire, motivate and empower others, whilst respecting diversity and celebrating the power that difference brings. Learn MoreFrequently Asked QuestionsNo, you don’t need to have any particular academic or professional qualifications to take part in the programme. We know that leadership development training can be very difficult to access in the social sector, so try to make
it accessible to those with minimal prior training. Your passion for and experience in the social sector are all you need to apply. We classify the leadership experience required to meet the programme eligibility as professional, in-sector experience. This may have been accrued at different points throughout your career, at different organisations and can be paid or voluntary. Social Prescribing Leadership applicants should be able to display at least three years of such experience. If your experience falls outside of these criteria, but you feel you are otherwise well suited for the programme, let us know why your experience is relevant and transferable in your application. If you have any questions, please email us at programmes@cloresocialleadership.org.uk. This email address is checked three times a week, so please expect a response time of 48 hours. As a Clore Social Fellow, you will join a large network of social sector leaders from across the UK. Opportunities to connect with local fellows and continue learning will be supported, encouraged and facilitated wherever possible.
We are always developing new programme offerings, and may have something of interest to you in the future, so keep in touch, and let us know if you have any ideas, or want to develop something with us for your organisation.
